Yes, and the ACA established a list of conditions they all had to cover. Before that, they excluded whatever they felt like excluding. I had an employer-based policy that did not cover pregnancy and delivery.

Most CS degrees don't have an ethics prerequisite & fewer still will be taught by a rightfully bitter prof who starts the class by making them watch Koyaanisqatsi, then follows that with coursework on Therac-25 & Cambridge Analytica. Whether those students even appreciate it is further questionable.
I think frequently about how many of my classmates and colleagues enthusiastically embodied the message "If you are good at computer / good at math, you don't want or need any ethical guidelines or boundaries"
