When I make a comic book at detective comics comics, I have minimum 3 but sometimes 4 sets of totally fresh eyes reading it out at various stages to catch things. I expect the news to at least meet the standard of care we give our fake stories full of fake people, if not exceed it.
The point of having a standards editor is that you bring in a skilled, disinterested person to read or watch a story and point out possible bias, errors and lapses.

The actual story editors can miss those things because they're so closely tied to the production and reporting of the piece.

Last night I watched this documentary about Lilith Fair & it made me a little sad. They created a little bubble of good vibes, love, tolerance, & safety, & for a while it was a beautiful thing, but the world is full of misogynist assholes who can't allow beauty to exist free of their control.

the entirety of the british press talked themselves into believing this woman is trans based almost entirely on JK Rowling's judgement of the athlete's appearance.
Nothing illustrates the arseholery of the IOC and the British Press more than the use of the image of a not-Trans woman to illustrate a story about trans women being banned (because there are no current trans women competing at this level)
