Answer hallucination (wrong fact), attribution hallucination(source didn't say it), citation hallucination(source doesn't exist)
I know there are many technical papers that try to disambiguate the use of "hallucinations" in RAG systems but here is my attempt. The third use case is a great worry for librarians (1)

Search A: prompt has typos in “association” & “blood clots.” AI output has medical misinfo (Yaz&Yasmin ARE NOT drospirenone-only). Search B: same words, same order, correct spelling. Output factually correct

Tallene er slående: Omtrent like mange betaler formuesskatt som lever under fattigdomsgrensen i Norge (~600 000). Likevel handler borgerlig valgkamp 2025 bare om den ene gruppa.

A beautiful pill scarab that I found in dead leaves in the Ecuadorian Amazon. Ceratocanthine are known for their ability to conglobate: rolling into a nearly perfect ball to avoid predators. They use the ball-mode even during inactivity - perhaps it's a way for them to rest too.

A beautiful pill scarab that I found in dead leaves in the Ecuadorian Amazon. Ceratocanthine are known for their ability to conglobate: rolling into a nearly perfect ball to avoid predators. They use the ball-mode even during inactivity - perhaps it's a way for them to rest too.
