The serum creatinine level is a concentration, amount per volume.

If you quickly expand plasma volume by 20%, the Cr will decrease by ~20%, saying nothing of GFR.

Same goes for ALT, WBC, CRP, and other values that we trend and sometimes overinterpret small fluctuations of.

Curious if folks ever use this in setting of rapidly rising creatinine to make med dosing decisions - why or why not?

In #DeepBrainStimulation

the two major developments across the last decade have been

1. Adaptive DBS and
2. Connectomic DBS

In a @natrevneurol.nature.com article with @julianneumann.bsky.social, we ask:

Could the two be united into a common framework?

The abdominojugular reflux test was performed by applying firm, sustained pressure to the center of the abdomen. A sustained increase in jugular venous pressure (JVP) of more than 3 cm of water for more than 10 seconds indicates a positive test. Full case details: nej.md/4752OVV

The ‘hippo’ in ‘hippopotamus’ and the ‘hippo’ in ‘hippocampus’ are the same.

‘Hippo’ is from the Greek for "horse."

Hippopotamus means “river horse.”

‘Hippocampus’ means “sea horse” because that part of the brain is shaped like a seahorse.

“but humans also make mistakes and AI will only get better”

if jane from accounting doesn’t know how to add she gets fired. AI has no accountability. it can make up a story about a person that, if a human spoke it, would count as slander. but since AI lacks accountability there is no recourse

1 bumet (PO&IV) = 20 torsemide (PO&IV) = 40 furos IV

1 dexamethasone = 5 pred = 25 hydrocort. PO = IV for all.

And I'll add one more:

1 lorazepam = 5 diazepam = 25 chlordiazepoxide. PO = IV for all.
(For posterity, the other conversions I think everyone should know in their head are loop diuretics and common corticosteroids)

Always look at the anion gap. Even if the bicarb is 24, there could be a lot of lactate or salicylate or whatever else in there, tug-of-warring with a metabolic alkalosis.

Huge energy bill!

1⃣Kilocalorie= energy to 🔥1L by 1°C
2⃣Ascites=Heating ~5-10L to 36°C 24hrs/day
3⃣Energy comes from body breakdown (sarcopenia)

If you⬇️ascites➡️the energy bill falls
