Potential confounders in studies identifying associations between plasma sodium correction rates and mortality. Na = sodium, AVP = arginine vasopressin A) A perceived causal relationship between an exposure and an outcome can be distorted by a third variable, known as a confounder. B) The serum sodium correction rate is the exposure, mortality is the outcome, and certain comorbidities and their severity act as confounders influencing both variables. C) Advanced stages cirrhosis, heart failure, and cancer are linked to higher mortality risk, sustained AVP release, and reduced kidney function, leading to impaired water excretion and slow serum sodium correction. D) In early stages of these conditions—or in their absence—patients exhibit better survival and, because hyponatremia is usually the result of reversible conditions with relatively preserved kidney function, serum sodium can rise rapidly through large-volume water diuresis.

Effects of Empagliflozin on Urine Biomarkers in EMPA-KIDNEY

Effect of empagliflozin versus placebo on study average concentration of urine biomarkers (indexed to urine creatinine). The MMRM model was adjusted for the prognostic variables specified in the minimized randomization algorithm (i.e., age, sex, prior diabetes, eGFR, uACR, and trial site region) in the same categories used in the minimization process, initial freezer storage temperature (-20°C versus below -20°C) and baseline log2-transformed urine biomarker-creatinine ratio. A weighted average of 2- and 18- month values proportional to the amount of time between visits was used to estimate the study average log2-transformed urine biomarker-to-creatinine ratio in each arm. Absolute values indicate geometric mean (approximate SE) creatinine-indexed urine biomarker concentration in the units specified. The size of the boxes represents the amount of information estimated.

Inflammatory Signaling and Potential Therapeutic Targets in CKD. Sterile inflammation triggered by innate immune system activation is a key driver of both chronic kidney disease (CKD) and associated atherosclerosis and cardiovascular disease (CVD), in part via activation of the NLRP3 inflammasome. Recent and ongoing clinical trials are testing therapeutics targeting the NLRP3 inflammasome, interleukin-1 (IL-1) and its receptor (IL-1R), interleukin-6 (IL-6) and its receptor (IL6R), as represented in this schematic.
