If you’re still ordering troponins “just in case,” you’re not risk-averse — you’re noise-generating.

#ManUtd not in for Evan Ferguson loan?
Surely with Rashford’s wage out we even could loan Ferguson AND Tel.

I’m going to repeat this sage advice; keeping supporting, keep watching but emotionally divorce yourself from this club right now. They’ve caused me/us way too much pain. Sure, wins don’t feel as good but loses don’t make me depressed anymore. We haven’t hit bottom yet, it’s going to get worse.

Yes it’s just for “companionship”; which is why it has that waist & those breast.. 👀

If they weren’t presenting it as a masterbation adjunct, it wouldn’t need to look like that.

In this day and age, living in a place that
- you like,
- you can afford,
- you can find a job,
- you have family/friends,
- you feel safe, and
- you *are* safe from natural disasters/bad laws/worse

is luxury and privilege. Almost no one gets to check all the boxes.

So maybe don’t be an asshole.

New research suggests that by the year 2030, NHS clinical staff will be spending more time completing e-learning than providing patient-facing care.

Laughable make-believe.
So this is the new great idea🤔

I’m pretty sure if Sarah had seen a GP she wouldn’t have needed to have a CT scan, no need to speak to the ENT registrar & yes it could be managed in the community

Why does NHS England always underestimate the skills of GPs?

So more work for private hospitals to cut waiting lists...the sector has many potential safety risks as government and NHS well know. Recommendations from the Ian Paterson inquiry have still not been implemented, leaving patients at risk. Read my 2023 piece here: www.thetimes.com/uk/article/y...
You can pay for private healthcare — but can you trust it?
More than a quarter of million people dipped into their savings or took a loan to pay for a private hospital operation or appointment last year — a record hig

Very very good piece on caring for the [potentially] dying patient.
While we’re on the subject of End of Life Care, this is a blog I wrote a while back, on looking after people who might be dying.

It was written with junior doctors in mind, to help them navigate waters that I had found tricky.

It’s not “becoming” dangerous; it’s been dangerous for a decade.
“Years of neglect and underinvestment left NHS buildings crumbling, staff forced to use outdated creaking equipment, and patients paying the price. It is unacceptable for patients to be treated in these conditions.”

Before you freak reflexively order IV hydralazine for that BP of 180/90, remember that people get BPs up to 480/350 (yes that is in mmHg) while weight lifting and don't "stroke out"

Have honestly just had to look up “beers list” meds, & I’ve been a prescriber since 2016 - very frequently clerking over-65 year olds.
Another 5% is coming to the independent realization that seemingly no one gives a shit about avoiding Beers list medications in elderly patients except geriatricians and PCPs. Especially if they’re admitted.

A different option to remote monitor arrhythmias in the future.

While wearable devices using photoplethysmography offer more convenience, they sacrifice accuracy. This study presents a RF based sensing system that monitors long-term HRV with clinical-grade accuracy
