You do get a feeling for pure Alzheimer's and vascular phenotype after seeing few hundred/ thousands of patients2/2
It’s not unusual to revise diagnoses of Vascular #Dementia in clinic, and replacing it with better diagnostic fit eg AD or Mixed. The study below suggests that diagnosis of VaD improves with experience.

Vascular dementia is consistently over diagnosed in my opinion, basically relying on radiology reports without actual review of images on PACS. I very rarely diagnose Va D, unless there are lots of vascular risk factors , falls, significant SVD , stroke or multiple lacunar infarct on CT. 1/2

My sister bought a new fridge with an ice maker, and she kept finding ice on the floor. A repair person came and he checked everything and could find nothing wrong, so he just sat in the kitchen and waited. Five minutes later the dog came running in, hit the ice button, grabbed a cube, and fled 🤣
