There was also damage to the body flap that shielded the engines on reentry. The launch pad sound suppression system wasn't good enough & it flexed well beyond specs.

I remember seeing this on the news. The blunt face of the OMS pods didn't get quite as much heating as the leading wing edges or the nose/belly area, so weren't as critical for tile loss - but yes, could have propagated to other tiles and led to a much worse outcome. Bonkers design.

Soviet 1VA No. 1, the first attempt to send a spacecraft to Venus, was launched #OTD in 1961. Due to a problem with the launch vehicle it failed to leave low Earth orbit and reentered the Earth's atmosphere on 26 February. It was officially named Tyazhely Sputnik (Heavy Satellite).
