When I did QA, this was something I yelled a lot at our devs. Placeholder should be visible and wrong, not 'close enough'.
A pre-aproved list of placeholders that is clearly communicated to everyone, is the best way to go. For Stakeholder builds if necessary, make changes build specific.

In gamedev, if you're using a placeholder asset, best practices involve marking it as placeholder so garishly and offensively clearly PLACEHOLDER that you can't possibly miss it. Also flagging that asset in the dev process so it can be identified quickly before the final build.
