These markers correlated with clinical success of OIT. In fact, we found that lower CD300b in monocytes and higher CD300f in eosinophils both correlate with a greater reduction in allergen-specific IgE.

Early in treatment, we observed a significant downregulation of the activating receptor CD300b across all myeloid cells. Simultaneously, the inhibitory receptor CD300f increased longitudinally, specifically on eosinophils.

The core finding of our research is that OIT shifts the balance of myeloid cells, including eosinophils, monocytes, and neutrophils from an "activating" state toward an "inhibitory" one by dynamically regulating CD300 receptors.

CASE NO. D L.Z. Aaed 37 This 37 vear old railway worker was admitted to hospital in October, 1973 with a one month history of anterior non-specific chest pain, fever and cough productive of moderate quantities of dark biood. Chest x-ray film revealed extensive, bilateral pulmonary infiltrates with possible cavitation. Bronchoscopic examination showed numerous white plaques on the mucosal surface from the larynx to the main bronchi. Haemoglobin was normal. There was moderate leucocytosis with eosinophilia and ESR greater than 100mm per hour. Biochemical profile was normal and no red cells were present in the urine. Subsequently the haemoglobin fell to 9gm% whilst the eosinophilia became more pronounced. Multiple, small haemorrhagic lesions appeared on both legs. Biopsy of the bronchial mucosa was reported to show squamous metaplasia with ulceration and fibrinous exudate. The lamina propria was densely infiltrated with inflammatory cells including numerous eosinophils.: Problems 1. What is the diagnosis? 2. What further investigations would you undertake? 3. Aged 39 Symptoms - Severe asthma since the age of 30. Born in England, well until age of 30 when breathlessness began over a period of one week. This required hospital admission and she was treated with high dose corticosteroid with improvement. Since then she has had multiple hospital admissions and has usually taken 10-15 mg of prednisone per dav. She is always mildly short of breath and has frequent episodes of worsening chest tightness with no known precipitating factor. She has never smoked. Past History - not significant Family History - No atopic disease, no asthma, no serious diseases. Clinical Findings - Facies revealed effects of corticosteroids but no weight gain or skin changes. Hyperinflated chest. Widespread rhonchi with decreased breath sounds. No eczema, nose normal. No sputum production. Chest Xray overinflation but no other abnormality. Investigations - Skin tests negative. Immunoglobulins including IgE normal. Lung Function : TLC = 120. FRC 160, RV 200 (percent predicted) FEV, .5L. DLCO - normal. BGTs PaO, 70; PaCO, 32; PH 7.42 Course - Admitted to hospital for high dose steroid therapy - IV decadron and IV theophylline and 32 aerosol by nebulisation. After 10 days FEV1/FVC of 0.6/1.2 --> 0.8/2.0 On discharge steroids reduced to 15 mgm of prednisone over a month and FEV1/FVC fell to 0.5/7.6 Problems 1. Does this woman have asthma? 2. Is there a place for bronchial biopsy? 3. What dose of corticosteroid should be used to test airway responsiveness and for how long? 4. What are optimal serum theophylline levels in such a patient?

It's for something called 'Fasenra'. From the sounds of things, it's an asthma thing that affects the eosinophils (Specific white blood cells that attack parasites & whatnot).

Remember, too, that urine eosinophils can be found in a variety of upper and lower urinary tract diseases as well, including infection, cholesterol microemboli, and papillary necrosis (see Nolan CR et al. However, eosinophils are not specific for allergic-type AIN as they may also be prominent in other acute and chronic diseases affecting the tubulointerstitium.

Prominent interstitial eosinophils (see arrow) are most often associated with allergic-type acute interstitial nephritis (AIN).

Just had a followup with my new pulmonologist. My eosinophils levels were high and I'm still getting asthma symptoms with the new medicine, so she's recommending I start injections to further control my asthma

Yes! I had to go visit the vampires yesterday for hematology next week and the email hit this morning and almost everything is shockingly normal for me! And the eosinophils were SUPER in range, I was pleasantly surprised.

I strongly suspect you will, because it works to reduce TSLP and that's a precursor to both eosinophils and IL-5 (which can increase eosinophils)
