In CKD, ⬇️renal clearance 👉 metformin accumulation

Metformin inhibits mitochondrial complex I& gluconeogenesis, ⬆️ pyruvate 👉 shifting metabolism toward lactate production

Compared with continuing metformin, stopping it was associated with a lower 3 yr survival (63.7% versus 70.5%; HR=1.26) while incidence of MACE was similar.
The contd use of metformin may be appropriate when eGFR falls <30 ml/min. RCT is needed to confirm these findings

None of the trials initiated at GFR below 20 which ties the gl writers hand
(Though flozins not stopped below 20)

Now DAPA-advanced CKD presented at #KidneyWk which started at GFRs 10-30 suggest slowing GFR >> GFR dip (paper not published yet)

RENAL LIFECYCLES awaited!
