The school I work at has the staff dress up, not the kids. Exactly for this reason. We have to explain who we've come as, and why we chose that character, and explain to the kids why we like the character and book they're from.

I think something relevant to how Britain perceives itself is that we, on the whole, tend to travel to the bits of Europe poorer than us.
Dutch here. Worked and lived in the UK for 10 years. Might I say this is wider than only healthcare. Even something as mundane as road maintenance. Always the ad hoc solution. Stingy about out of pocket costs. In the end total costs are higher than structurally trying to prevent the potholes.
