This seems like a - let us write more guidelines that will get heavily cited and boost my h-index - piece

I will argue we need less guidelines

They should be like wars - waged only when absolutely necessary (stealing from @cardiobrief.bsky.social Larry Husten www.forbes.com/sites/larryh...)
Why Health Guidelines Should Be Waged Like War
Updated Here's a modest proposal: we need fewer and shorter guidelines. In fact, I'd like to propose that guidelines, like war, should be waged only when there is absolute consensus and overwhelming e...

Great work as ever from all involved. mild dismay at infiltration of “… up to x%” instead of “approximately x%” from general journalism in to nephrologist prose. up to x = 0 to x 🫣

ACEi and ARBs are not “nephrotoxic.”

We all know about their long term renal protective benefits…

But even in the short term, they improve renal blood flow!

They do reduce GFR and impair K+ excretion, which is why we might hold them for a bit in AKI.

But bad for kidneys? No
Good morning to everyone except the Walgreen pharmacist telling my patients that lisinopril is bad for the kidneys.

I've never been to an M&M about kidney injury attributable to iodinated contrast.

I've been to several M&Ms for adverse outcomes from missed/delayed diagnoses because people hesitated to use IV contrast for fear of what we now know to be a ghost
So without a randomized clinical trial this is what we are left with. The highest quality epidemiology studies are unable to detect any increase in AKI, so those bumps in creatinine following contrast are almost certainly run of the mill AKI and not contrast induced AKI.
