In the CONFIDENCE trial in persons with chronic kidney disease and type 2 diabetes, combination therapy with finerenone and empagliflozin led to a greater reduction in the urinary albumin-to-creatinine ratio than either drug alone. Full trial results: nej.md/4jEVbbk

just learned this & mind blown 🤯

CKD-EPI, MDRD etc give us GFR corrected for body surface area (ml/min/1.73m2)

BUT we're supposed to use the *absolute* GFR (ml/min) for drug dosing (eg KDIGO 2024 guidelines below; FDA agrees)

in obesity, the computer's ml/min/1.73m2 can be way too low #EMIMCC

Hypochloremia is associated with diuretic resistance. In 👇 study from Yale, hypochloremia was associated with poor diuretic efficiency (defined as mmol of UNa per doubling of loop diuretic dose). Of note - serum sodium was NOT associated with diuretic efficiency #cardiosky
