Finally, the classically recommended ‘4-to-6-week’ regimen of either FQs or TMP-SMX is not based on solid evidence, as no studies directly assessing 5 or 6 weeks of these Abx. Only fosfo was studied for durations over 4 weeks.

In critically ill pts, avoid adjusting initial doses based solely on early bloodwork. When pts present to ED, SCr/eGFR may not accurately reflect kidney fxn. These early labs represent pre-treatment📷(before IV💧, Abxs & resus). W/ these interventions, SCr/eGFR often improves #emimcc #medsky #pharmsky
