TBH diuretic infusions are dumb

for diureric-resistant patients, you need high-dose bolus diuretics to achieve concentrations high enough to cause diuresis

for diuretic-sensitive patients, it doesnt matter (either strategy will work fine)

Puppies (even old man puppies) don't understand time change 😭. Idk if dogs experience cold diuresis like humans do, but they can get diabetes (which is linked), so I imagine they do. And it is getting chilly.

She lived to 17 when she was diagnosed with kidney failure at 13. When she was diagnosed with heart failure (I brought her to the vet ER for respiratory distress) at 17 they said I could admit her for diuresis. But I knew she would loathe it. It was time.

Here are some practical tips for the hospitalist taking care of patients with HCM:

- Take care to avoid tachycardia, hypovolemia, and sudden drops in afterload.

- Practically, this means maintaining hydration, being more ginger with diuresis, avoid vasodilators, control pain/anxiety and fevers

Well bummer. Daily assessments is a bit much. I think we overthink this. The best use of Pocus is to distinguish cardiogenic from noncardiogenic edema. I really just use it to try and predict whether diuresis is going to help them breath better. Don’t need it daily, just to pick a stategy

Vena cava is good when people start to ask whether decreasing diuresis an rising kreatinin is because they are dry. More furosemide is most often the solution

Several studies using LUS to guide diuresis have been positive, and it’s just as simple to look at, and has also been shown to be a good surrogate for LAP (though VeXUS plus LUS plus LV function and VTI is my own practice to guide decision making). This was poorly thought out imo.

Yes youre correct! Even in healthy people lying flat can induce a (mild, insignificant, non problematic) diuresis. To my mind this paper takes it a step further by suggesting the upright position helps block water losses - at least in MSA patients with autonomic failure

Some key findings:
Dysautonomia patients had no diuresis in response to the water load when standing, but normal diuresis when lying down

During water deprivation, at night (lying down) dysautonomia patients ↑ sodium & potassium excretion & ↑ urine volume

Medically, that procedure is an extremely big deal, he would have to briefly at least turn over the presidency to the VP. More likely he had some type of imaging or IV diuresis and or I wouldn’t even be surprised if somebody said he had dialysis, but I’ve not seen his record so I don’t know.

Heart failure exacerbation on top of vascular dementia is a bitch.

Totally makes sense that he went awol for 5 days to get aggressive diuresis to pull off all that cankle fluid.

Need some pics of his calves and hands to verify, but that’s my bet.
