Me [ordering a CBC, BMP, LFTs, CRP, ESR, ANA, pro calcitonin, CXR, EKG, CT-PE, d-dimer, lipase, BCx, UA]: when it comes down to it, 90% of diagnosis is taking a thorough history.

I'm a Lung Dude from my Advanced Training
We frequently see residual changes on a CXR at 6-8 weeks after a pneumonia. It's sterile, but persistent inflammation, clearing slowly. Pathology 101, acute inflammation takes this long to heal

esp. for a CXR that often takes less than a minute from opening it to signing the report. if the prelim report is subpar and I have to read, move back-and-forth between image and text and redact it, it would probably slow me down, leading to not adopting the tool. the improvement must be substantial

Take-home message:
Postoperative cardiac surgery patients can suffer multiple catastrophic complications. POCUS findings can be very helpful but should not be over-read. In this case, even in the absence of lung sliding, the chances of tension PTX without significant CXR findings

Writing a reflective peice on CXR diagnosis and cardiogenic pulmonary oedema. Came across this.
