Though "Oral is the New IV" with antibiotics (h/t @bradspellberg.bsky.social), it's not always easy to convince reluctant inpatient clinicians -- especially (in my experience) surgeons, who want to treat "aggressively".

The US military is the most vaccinated segment of the population. They achieved 97% COVID vaccine coverage in 2021 and receive special vaccines pre-employment in addition to standard adult vaccines.
We should aspire to the same levels of vaccination coverage for children in US not less.

This is discussed in wikiguidelines. It is reasonable to not reset if the valve culture from the explant is negative, and it is reasonable to start over if the culture is positive. But there are no data really either way. See WikiGuidelines answer below.
