In the past month and a half in FGO, I have been spooked by 5* Rider servants three times.
First time happened when I tried for Maou Nobu. I got Achilles instead.
Then, a few weeks later, I tried for a second copy of Okitan Alter. Achilles showed up again.
I didn't have him at all before this.
