1) the name, board specialty, and license number of the doctor making the determination the treatment was not medically necessary;

This is painful to write; the fact of the matter is that as destructive as Donald Trump is, he has kicked the ass of everyone who got in his way...because when he comes to a "fight"...he understands it's mother*cking FIGHT, and he understands that in a fight, the primary goal must ALWAYS be to WIN.
