The BICAR-ICU-2 trial just dropped!

RCT involving IV bicarb in metabolic acidosis among patients with mod-severe AKI

Bicarb didn't affect mortality but it reduced the need for dialysis & risk of bacteremia

VERY similar conclusions to BICAR-ICU-1

Caveat: some meds are designed with certain pharmacokinetic goals in mind. Aminoglycosides, for example generally work better with higher peak concentrations, so splitting the dose up is counterproductive despite achieving a smoother concentration

In Lupus nephritis patients, neutrophils have increased surface LAMP1, which is externalized after activation. Soluble LAMP1 is increased both in the blood and urine of patients, correlating with histological activity. #NephSky 🧪

Yes you should be able to reduce medications after adding Spironolactone.
I withdraw based on other adverse med effects (edema = CCB; K = thiazide/RAASi)
