Anyone who has ever been in the military knows that if it happens on your watch, you own it. It was YOUR responsibility. No Commanding Officer wants to hear jack shit about what the guy on the watch before you did if the event in question happened on YOUR watch. So, thank you, DJT, for $7 a dz eggs.

The DC control tower was understaffed. There was only one controller, controlling both helicopters and planes. Those are supposed to be separate jobs.
