Step one of design for me occurs when a weird spot randomly appears in my brain & starts growing, like mold.
Other spots will eventually appear & grow until my entire brain is consumed.
Writing it is much the same. Starts with random bits that grow until they finally overlap to make a full game.

When I write a draft of a new TTRPG I like to start with a table of contents. This is because:
- I like having a to-do list
- I am bad at organization and starting there helps me figure it out
- It lets me jump around the doc without losing sight of what's already there
