Accueillir les demandes de transition sans stigmatisation et proposer un accompagnement adapté En fonction de la situation personnelle de la personne trans qui s’engage dans une démarche de transition médicale et de l’état d’avancement de sa réflexion, ses besoins varient. C’est pourquoi la HAS recommande de personnaliser la prise en charge et d’adapter l’accompagnement psychosocial tout au long du parcours. L’enjeu est de favoriser l’autonomie de la personne afin de l’aider à identifier et formuler des choix éclairés, dans le respect du principe d’autodétermination. Il est essentiel d’accueillir la personne sans jugement ou idée préconçue quant à son identité de genre et ses besoins, en utilisant notamment le prénom et le pronom demandés. L'identité de genre ne doit pas faire l'objet d'une évaluation psychiatrique spécifique. A l’image de la diversité des situations et besoins, la HAS recommande une prise en charge globale et pluriprofessionnelle, mise en œuvre par une équipe de soins. Les recommandations sont destinées à un large panel de professionnels : médecins généralistes, endocrinologues, psychiatres, psychologues, médecins de la fertilité et de la reproduction, gynécologues-obstétriciens, urologues, chirurgiens plasticiens, médecins du travail, travailleurs sociaux. La HAS préconise que le médecin généraliste puisse accueillir toute demande de soins et coordonner la prise en charge tout au long du parcours. Il peut assurer le suivi d'une prescription et, s'il est formé, la primo-prescription des traitements hormonaux d'affirmation de genre. Les recommandations insistent sur l’importance de l’entretien initial avec le médecin généraliste ou un professionnel d'une structure de premier recours. Eclairer les demandes de prescription d'hormones et de chirurgie La prescription d’hormones aux personnes qui la demandent peut contribuer à améliorer leur qualité de vie. Une évaluation personnalisée des bénéfices et des risques est recommandée, en recherchant notamment des facteurs de risques (vasculaires, oncologiques, cardiologiques…). Concernant la prise en charge chirurgicale, la HAS recommande de répondre aux demandes de chirurgie des personnes trans. Elle souligne la nécessité de délivrer une information préopératoire claire, loyale et adaptée sur les modalités chirurgicales, les risques (à court ou moyen terme) et le caractère irréversible de certains actes pour lesquels un délai de réflexion est prévu, afin de permettre à la personne de donner son consentement éclairé. La HAS attire enfin l’attention des pouvoirs publics sur la nécessité de renforcer la formation initiale et continue des professionnels, de structurer l’offre de soins et de mettre en place une organisation territoriale adaptée aux besoins d’accompagnement et de prise en charge médicale et chirurgicale des personnes trans

a presidential term is 4 years. between 14 & 18, i tried to die 4 times because i couldn't stop puberty. i got addicted to drugs. sexually assaulted. had an eating disorder. dropped out of college. i was unhoused.

i survived by accident.

Does "women and femme" include feminine men? Or is that a code to include trans women without saying the "women" part excludes them?
