Dog keeps asking to go out in garden for a wee.
As soon as he is out, and realises how cold and wet it is, he turns round and comes straight back without bothering to wee.

Paracetamol (Tylenol) is one of the most effective and safe painkillers ever discovered. Keep within the recommended dose. Take it in pregnancy if you need to. It has been shown in MANY studies not to cause autism. 🧪
Had toothache today. Took paracetamol. Pain disappeared. I did not become autistic.

Just read this book by @hayleygullen.com with forward by @lizoriordan.bsky.social . Not a graphic novel person normally but it is great and so bluntly true to my experiences. Highly recommend the book, not having breast cancer or some of the tough NHS experiences

If someone had told me 20 years ago that one day l’d be number 1 in breast cancer on Amazon I’d… probably not have been too happy about it.

Still, this is nice to see, today!

Dear parents - Yes you can give your sick child paracetamol if they’re >3 months old. It’s an excellent painkiller. It brings down a high temperature. It’s one of the safest drugs available. You are right to want to soothe your child’s distress. Evidence supports your good parenting. Be confident.

As a GP of 35 years the more I learn the more I realise I don’t know
meanwhile, even an experienced expert generalist doctor like say an A&E consultant or Acute Internal Medicine is not able to work in general practice seeing undifferentiated patients cos they are not a qualified GP

Well I think this is really good, particularly if you know the pt, or in their presence for a while eg on the ward and observe the BES
I'm no longer interested in hearing about the patient's NEWS.

Give me their VIBES instead...

V – Vitals (HR, RR, BP, Temp, Sats)

I – Inspired oxygen

B – Behaviour (alertness, agitation, new confusion)

E – Escalation risk (triggers for concern)

S – Stability (improving, static, deteriorating)

Full day 9hrs 27 face to face appts - 1hr lunch break (no golf)

10yrs
9hrs 27 f2f + 7 tele - 30mins lunch

20yrs
9.5hrs 28 f2f + 12 tele - 15mins lunch

30yrs
10hrs 40-50 mixed F2F/tele/econsults - lunch at desk - extra 1 hr home

I couldn’t do it anymore
