Every election is a yes or no decision no matter how many options are on the ballot. Options are selected in the primary election. The general election determines control.
America has a 2-party system at the federal level. There are no sizable 3rd parties that can provide an option.

I realize there's a temptation to settle on some simple explanation, but I don't see how it's the voters' fault for having only one option on the ballot for a particular election. That happens to a LOT of districts in my red state with no Dem running in the race!
