A 25-year-old pregnant woman presented with a mild, persistent frontal headache. She was diagnosed with macroprolactinoma 3 years prior and was treated with bromocriptine. Neurological examination was unremarkable, and interestingly, visual field testing remained normal. Pituitary magnetic resonance imaging demonstrated an enlarged, cerebrospinal fluid-filled sella turcica consistent with partial empty sella. The optic chiasm was displaced inferiorly, herniating into the sellar cavity (A and B, white arrows).

The answer is B, bromocriptine. The dopaminergic agent and prolactin inhibitor can cause vasospasm with serious consequences such as myocardial infarction and stroke.

Oh wow. Well, brilliant that yours has shrunk this way. And yes, totally agree on the Cabergoline! I was on bromocriptine initially and didn’t tolerate it. Hope yours will continue to shrink!!

Oh hey, much empathy from me. I had one for a decade. The first meds (bromocriptine) made me super sick and did absolutely nothing; the second set (cabergoline) took it away entirely. Really hope you can get rid of that f*cker and feel better.

What do Parkinson’s disease, LSD, and (likely) the ancient beverage kykeon have in common? Ergot, a barley fungus which can cause hallucinations when consumed.

Ergot dopamine receptor agonists include drugs like bromocriptine, cabergoline (non-ergot include pramipexole, ropinirole, rotigotine)
