Senate committee holds hearing on physician burnout
Expert witnesses discussed the impact of federal regulations and administrative burden on physician burnout in a recent hearing held by the U.S. Senate Special Committee on Aging. “Today, we will look at how Washington’s regulations and red tape play into this crisis, and what we can do to fix it so our doctors can spend more time caring for patients and less time navigating bureaucracy,” committee chairman Sen. Rick Scott (R-Fla.), said in his opening statement. “We’ll hear from witnesses who interact with physicians at all levels.” Key points of discussion included difficulties managing

Reduced haemorrhage during external medial orbital wall decompression with nasal packing containing vasoconstrictive agent
Objective In retrocaruncular medial decompression, the incision is small relative to the depth of the surgical space, and haemorrhage may obscure vision. Measures that minimise bleeding are most important to improve visualisation and facilitate the procedure. Despite this, there is no consensus regarding the effect of nasal packing with a vasoconstrictive agent. The aim of this study was to see if haemorrhage during external medial decompression could be reduced by nasal packing. Methods and analysis A randomised, non-blinded, prospective study in which patients listed for orbital medial wall decompression at St Erik Eye Hospital, Sweden, were recruited. Patients were randomised to receive either nasal packing with a 4%+0.02% Cocaine Hydrochloride-Adrenaline solution or the control group. Intraoperative bleeding was quantified by weighing the suction device before and after the procedure. Results A total of 46 medial orbital wall decompressions in 37 patients were included in the final analysis. The treatment group, which received nasal packing, demonstrated significantly reduced intraoperative bleeding compared with the control group (median 9.40 g vs 30.10 g, p

Early vitrectomy for macular retinoschisis after anti-VEGF therapy in aggressive retinopathy of prematurity
A female infant (gestational age 31 weeks, birth weight 1350 g) with aggressive retinopathy of prematurity (A-ROP) developed severe posterior vitreous contraction and traction, a vision-threatening complication known as the “crunch” effect, two weeks after intravitreal ranibizumab injection (A, E). Optical coherence tomography (OCT) demonstrated macular retinoschisis rather than detachment (B, F). At 40 weeks postmenstrual age, the infant underwent pars plana vitrectomy (PPV) during which a complete posterior vitreous detachment (PVD) was induced, followed by an intraoperative anti-VEGF reinjection at the conclusion of surgery.

Stability of Astigmatism following Lensectomy for Pediatric Cataract
Following primary pediatric intraocular lens (IOL) implantation, the goal is to have sufficient hyperopic refractive error that as the child ages the resultant refractive error allows reasonable uncorrected visual acuity. In addition, there are limited data on the natural history of astigmatism following pediatric lensectomy. With the increasing use of toric intraocular lenses (IOLs) in children undergoing cataract surgery,1-3.it is important to understand changes in astigmatism over time in a child’s pseudophakic eye.

Regulatory Paradoxes and Unnecessary Waste
Five years ago, our multisociety survey of North American cataract surgeons’ practices and opinions regarding operating room waste highlighted the need for ophthalmology to reduce carbon emissions and waste.1,2 Since then, discourse, research, and engagement around sustainability in eye care have increased markedly. Much of this has been chronicled and shared through the website of EyeSustain (www.eyesustain.org), a global coalition of 55 societies committed to advancing sustainability in ophthalmology through education, research, innovation, and advocacy.

This Issue at a Glance
Jabs et al (p. 310) conducted a randomized comparative effectiveness trial to compare adalimumab to conventional immunosuppressive drugs for treating noninfectious uveitis. Analysis of 227 participants showed that there was a significantly greater cumulative proportion of participants with successful corticosteroid sparing in the adalimumab group at 6 months but not 12 months compared to conventional immunosuppression. Adalimumab also resulted in a significantly greater cumulative proportion of individuals with successful corticosteroid discontinuation by 12 months compared to conventional immunosuppression.
