Axpaxli meets primary endpoint vs. aflibercept in wet AMD
In a phase 3 trial, Axpaxli, a tyrosine kinase inhibitor, achieved its superiority primary endpoint vs. aflibercept for the treatment of wet age-related macular degeneration, according to a press release from Ocular Therapeutix. “Despite advances in [neovascular age-related macular degeneration] treatment, there remains unpredictability around patient response to treatment, resulting in unsustainable burden and lack of adherence,” Pravin U. Financial hardship has significant impact on cancer-related pain
Patients who have financial hardships have a significantly higher likelihood of experiencing moderate or severe cancer-related pain than those who do not. A retrospective analysis also showed patients with financial hardship have higher rates of opioid use, despite their fiscal condition. “Financial hardship is clinically impactful,” Courtney P. Williams, DrPH, assistant professor in the division of General Internal Medicine and Population Science at The University of Alabama at Birmingham (UAB) Heersink School of Medicine and associate scientist at O’Neal Comprehensive Cancer Center, told

A series of ruptured dermoid cysts in pediatric patients
Ruptured dermoid cysts can often have atypical presentations and be challenging to manage. We report 3 cases of pediatric patients with ruptured dermoid cysts. Case 1 was a 16-year-old boy with an enlarging left superotemporal orbital mass who presented with 2 days of acute inflammation. Computed tomography revealed a well-circumscribed lesion that was excised. Histopathology confirmed a ruptured dermoid cyst. Case 2 was a 4-year-old girl with a right superotemporal orbital mass that enlarged following trauma.

Aqueous humour outflow improvement after excimer laser trabeculostomy
The purpose of the study was to investigate changes in aqueous humour outflow (AHO) after the Elios glaucoma procedure (excimer laser trabeculostomy, ELT) using sequential AHO angiography. AHO angiography using two different tracers was performed in six eyes of five patients undergoing cataract surgery combined with ELT. AHO was compared in regions of interest in the nasal quadrant. AHO improved significantly after ELT (p=0.03) overall, and each patient showed individual, qualitative AHO improvement. Interestingly, AHO improvement extended beyond the site of ELT application. Further studies are needed to investigate the correlation of AHO improvement and clinical efficacy.

Subjective and objective refraction using autorefraction and two wavefront-based aberrometers in EDOF and simultaneous vision intraocular lenses
Accurate assessment of refractive error remains a cornerstone in ophthalmic practice. The current gold standard is subjective refraction (SR), as it is considered to provide the most precise refractive status. Autorefractors have been routinely used in clinics to facilitate efficient daily ophthalmic workflow, offering a relatively rapid assessment of objective refraction (OR). In recent years, multifunctional diagnostic devices have gained considerable popularity. These systems combine OR with additional modalities such as aberrometry, tomo- and/or topography.

Mismatch Repair Deficiency in Benign and Atypical Ocular Sebaceous Neoplasms: Implications for Muir–Torre Screening and Classification
Muir–Torre syndrome (MTS), a variant of Lynch syndrome characterized by visceral malignancies and cutaneous sebaceous neoplasms, is defined by germline pathogenic variants in mismatch repair (MMR) genes. These Lynch-associated cancers include colorectal, endometrial, ovarian, and small bowel cancers, as well as tumors of the urinary tract and biliary tract.1 The most common sebaceous neoplasm in the periocular region is conventional poorly differentiated sebaceous carcinoma, a high-grade basaloid carcinoma usually without any mature sebocytic differentiation and frequently showing TP53 mutation and/or RB1 loss, which is generally not related to MMR deficiency or MTS.

Ophthalmologists: Encourage use of protective eye wear in sports
Sports-related injuries are extremely common in every eye doctor’s practice. These injuries are more common in younger patients between the ages of 10 and 19 years, in male patients, during competition vs. practice and especially when protective eye wear is not used. For example, 97% of injuries in basketball, baseball/softball and fishing occur in individuals not wearing protective eye wear. The common causes include trauma from balls, hands/fingers, heads, knees, elbows, rackets/sticks and fishing lures/rods. Common injuries, in elevating severity, include periocular contusion,

Expert witnesses discussed the impact of federal regulations and administrative burden on physician burnout in a recent hearing held by the U.S. Senate Special Committee on Aging. “Today, we will look at how Washington’s regulations and red tape play into this crisis, and what we can do to fix it so our doctors can spend more time caring for patients and less time navigating bureaucracy,” committee chairman Sen. Reduced haemorrhage during external medial orbital wall decompression with nasal packing containing vasoconstrictive agent
Objective In retrocaruncular medial decompression, the incision is small relative to the depth of the surgical space, and haemorrhage may obscure vision. Measures that minimise bleeding are most important to improve visualisation and facilitate the procedure. Despite this, there is no consensus regarding the effect of nasal packing with a vasoconstrictive agent. The aim of this study was to see if haemorrhage during external medial decompression could be reduced by nasal packing. Methods and analysis A randomised, non-blinded, prospective study in which patients listed for orbital medial wall decompression at St Erik Eye Hospital, Sweden, were recruited. Patients were randomised to receive either nasal packing with a 4%+0.02% Cocaine Hydrochloride-Adrenaline solution or the control group. Intraoperative bleeding was quantified by weighing the suction device before and after the procedure. Results A total of 46 medial orbital wall decompressions in 37 patients were included in the final analysis. The treatment group, which received nasal packing, demonstrated significantly reduced intraoperative bleeding compared with the control group (median 9.40 g vs 30.10 g, p
