In November 2023, the UK Health Security Agency was notified of PCR-confirmed group B (MenB) invasive meningococcal disease (IMD) in a 3-year-old child (Case A), followed by probable IMD in a 2-year-old (Case B, culture and PCR tests negative) attending the same nursery. An incident management team (IMT) was convened. Both children were fully vaccinated with the MenB vaccine 4CMenB (Bexsero, GSK Biologicals). All 39 children attending the nursery and nine staff received ciprofloxacin chemoprophylaxis preceded by pharyngeal swabbing. Pharyngeal swabbing yielded two MenB isolates matching Case A. Antibiotic sensitivity testing and assessment of 4CMenB vaccine coverage using the meningococcal antigen typing system (MATS) revealed the strain was not covered by the 4CMenB vaccine. Although the alternative MenB vaccine, MenB-fHbp (Trumenba, Pfizer), is only licensed from 10 years and has never been given to children previously immunised with 4CMenB, the IMT considered the benefits of outbreak control outweighed potential risks. Two doses were given 4 weeks apart to 38 children (one family declined) and all staff; there were no serious adverse events. Our findings highlight the utility of swabbing to identify outbreak strains and provide first evidence for safe use of the MenB-fHbp vaccine in children previously vaccinated with 4CMenB.

This is why MenB is used for short-term risk reduction (outbreaks, specific exposures), not long-term population immunity and why routine boosters aren’t broadly recommended unless risk persists.

MenB is even more complicated. MenB vaccines induce narrower, strain-specific responses, rely on protein antigens with variable expression, show faster waning of functional protection (often within 1–2 years).

Curious about MenACYW and MenB ... how convinced are we that effectiveness wanes? Not just titers, as with other shots titers drop and don't mean lack of immunity since we expect memory B-cells etc to still protect?

Today's report covers April 2025 recommendations from ACIP on the newest MenABCWY vaccine: it can be used instead of giving two vaccines (MenACWY plus MenB) for:

✅ high risk kids ≥10 yrs who need both ACWY and B coverage
✅ healthy people 16–23 yrs who want both ACWY and B coverage per shared CDM

Prior to 2023, the US had 4 licensed meningitis vaccines: 2 covered serogroups A, C, W, and Y (MenACWY vaccine) and 2 covered serogroup B (MenB vaccine).

In 2023, the FDA licensed two vaccines that cover A, C, W, Y and B (MenABCWY vaccine).

and in the US, the disease is rarer here, and mostly impacts teens/young adults. also, there are some studies on how immunity of menB given in infancy wanes by the time they hit the higher risk years, so by waiting, they're hoping to get it when most effective

I believe a major factor of the UK's decisions on vaccination involves total cost, due to the NHS. So, they do more cost analyses in general. intiitally, the menB vaxx there was too expensive but then they got a much better rate from the manufacturer which made infant vaccination more cost effective

So where does the “wait 2 weeks” advice come from? Often from older COVID-era caution, local clinic policy, or concern about attributing AEs not from evidence that immune responses are impaired. There’s no data showing MenB or MenACWY⬇️ responses to flu or COVID vaccines if given closer together.
