Obvious to my mind is the immune system and inflammation, including mast cell activation and (iirc) prostaglandin inhibition (which all links to glucocorticoid action)

What I was less pleased to see is the lack of discussion of *other* possible mechanisms, with the only mention I could find being this:

"but it is possible dexamethasone may be having a pharmacological effect independent of endogenous glucocorticoid action"

The mechanism proposed is one I'd never heard of: Local endometrial glucocorticoid deficiency

As such, the study aimed to replace that deficiency with dexamethasone. I was pleased to see in the discussion consideration for local (rather than oral/systemic) administration

Glucocorticoids activate glucocorticoid receptors but also mineralcorticoid receptors. Recently vamorolone was approved as an alternative steroid for Duchenne. Vamorolone binds less to the glucocorticoid receptor and thus does not translocate to the cell nucleus.

I contracted COVID-19 in 2021. In 2022, I was diagnosed with APS and lupus, in addition to pre-existing VWF type 2 and type 5 abnormalities and a 40-year history of adrenal insufficiency.
I suspect chronic inflammation related to glucocorticoid dysfunction contributes to vascular damage.

As a patient with APS/lupus, VWF2 and 5 abnormalities, and 40 years of confirmed adrenal insufficiency, I suspect that not only viral but also bacterial inflammation, along with glucocorticoid dysfunction, contribute to blood vessel damage.
