I have no need of that hypothesis.
That's certainly one option. Another (maybe simpler) is to use much more informative outcomes than dichotomised responses and time to first event.

Couple of quick responses - yes rate diseases are a BIG problem as it's really hard to do trials in a conventional way because of the lack of numbers. But that's not an excuse to allow sloppy thinking and inadequate research

And of course lots of hospitals are in locations that aren't easy to reach by public transport.
This sort of exploitation seems to have been completely normalised.

Absolutely! The whole idea of employers charging their "valued" staff to come to work is insane. Worst example in UK is hospitals, where doctors and nurses and everyone else are expected to pay through the nose for a parking permit.
