This brings us to the "simplified" risk-of-bias tool that doesn't have an actual problem to solve, nor add anything. But why not gather friends, throw opinions around, load up the CV, and then lament the continued explosion of low quality systematic reviews.

FRESH-UP is my favorite trial of 2025 so far (despite some superiority design limitations). My takeaway:
In patients with chronic HF, liberalizing fluid intake vs restricting <1.5L doesn't worsen HF symptoms or require higher diuretic doses, among euvolemic patients on good GDMT

What a journey this paper took.

Hydroxychloroquine was the first (false) hope of the pandemic. Trump touted it and took it himself. Even my hospital at the time, Yale, jumped on the bandwagon, briefly using it.

But researchers and patients generated RCT data showing it doesn’t work for COVID.
