A little goes a long way, and just by bringing up advanced care planning and asking a few questions about their preference even without a specific framework you‘ll probably deliver more EOL care than many other centers

The trend in the US was to start at higher GFRs throughout the 2000s and this peaked in 2010 and since then there has been a small decline in mean GFR at initiation but imo it is still too high.

Summary: urinary anion gap is not a good measure of urine ammonium and should be abandoned.

If you need to know the urinary ammonium, measure the urinary ammonium.
