My Mam got home from hospital Wednesday tea-time 🙌
Since then I’ve referred her to the district nurses… the re-enablement team at home.. arranged her follow up out patients appointment… and booked patient transport for the appointment.
She’s had OT and district nurse visit which is brill 👏🏻
