You could honestly boil this down to even fewer bullets, IMO:
1. Failure to delegate (this covers all of the "approval" or "direction" issues)
2. Not prioritizing retention (turnover has *enormous* hidden costs)
3. Lack of transparency (this is where weak direction and decision-making live)
