OpenAI is exploring “consumer health tools”.
If an AI is giving health advice, that’s medicine at scale.

And if it’s medicine, three things are non-negotiable:

1️⃣ Doctors in the loop
2️⃣ Older people in the design – not as an afterthought
3️⃣ Data that actually contains older people

The system isn’t broken. It’s obsolete.

We’re not overwhelmed by ageing.
We’re paying the price for decades of lazy design.

The healthcare system was never built for older people.
It was built for short-term fixes, single diseases, and people who recover fast.

If you read (social) media about AI, you almost become afraid of what AI will do to healthcare.
Personally, I’m more afraid of what will become of care without AI.

Despite the hype, the mistakes, the hallucinations, the threats I believe AI can make care better.

❓ What do you think?

🧠 "#AI not one-size-fits-all”?
It’s not even one thing!

#AI is many has many faces:
🧮 Predictive models: Structured data — labs, claims, vitals — outcomes, stratify risk,.
🗣️ NLP, LLMs, and Generative AI: Messy, human language — summarize, interpret, engage

When doctors want to stop medication, wefocus on the clinical reasoning: Risk of bleeding. Limited benefit. A burden.
✔️ All true — but not enough.

Help, my AI has a delirium 😵💫

Last week - as a test - I asked an AI "who is Simon Mooijaart?" It gave me a confident, detailed answer... about someone else entirely 🙃

As a geriatrician, this instantly reminded me of delirium - a condition I see daily at the bedside: #delirium

Well said. Not including older adults in research studies about diseases that primarily affect older adults is just ageism, pure and simple.

☑️ The original CAM does work when used by trained raters

‼️ But an unvalidated CAM variant - with no cognitive testing/interview - is widely used in clinical practice

⚠️ This 'CAM-Lite' has dangerously low sensitivity

Use of #AI decreases with increasing age.

#AI offers a lot of promises also in care for older people
#AI is not tailored to needs of older people

Systemic treatment among frail older patients with cancer: An observational cohort
In the past, certain oncological therapies were not offered to frail older patients. However, the advancement of geriatric oncology, tailored chemotherapy regimens, the introduction of new treatments, and the optimization of supportive care have contributed to enhancing the therapeutic margin. We aimed to evaluate the benefit of systemic treatment among older adults by assessing the three-month survival of older frail patients with metastatic cancer.
