1/n sharing a case: Young adult comes in 4 elective ENT procedure. Receives several doses of ketotolac for 3-4 days + empiric vancomycin. Then, labs are checked: Cr 4.0, up from 1.0. UA: neg blood/protein/cells. High suspicion 4 IV NSAID / IV Vanco hemodynamic/tubular insult. Non-oliguric, lytes OK

I've always assumed that people who drink ethylene glycol do it because they've internally asked themselves, "I wonder what it's like to have primary hyperoxaluria, types 1, 2, or 3?"
Ethylene glycol intoxication presents w/ anion gap metabolic acidosis & acute oxalate tubulopathy. Detection of urinary calcium oxalate monohydrate crystals helps in diagnosis [not classic dumbbell nor envelope shaped (dihydrate) but rather “cigar” shaped]. Slide courtesy of @jrseltzer.bsky.social

Fun fact: cystinuria was the first of the genetic kidney stone diseases to be characterized. Garrod presented his findings at the 1908 Croonian Lectures in London. Primary hyperoxaluria was the next to be described, and that was in 1925 by Lepoutre.
Cystine crystals (bright field, x400).

Their presence in #urinarysediment is pathognomonic for #cystinuria, the most common kidney stone disease with Mendelian genetics.

While perfectly specific, the sensitivity of cystine crystalluria is quite low.
