because this old post seems to be resurfacing—here’s my advice if you get an insurance claim/pre-auth denied. seriously. go straight to the government. your state has a dept of insurance. file a complaint with them. email your state representative too. they will do something about it.
my tip is that asking for the documents used to make the denial determination won’t actually do anything 99.9% of the time and is a waste of time tbh :( go straight to the top and file a complaint with your local dept of insurance/dept of health. they actually scare insurance companies

1) the name, board specialty, and license number of the doctor making the determination the treatment was not medically necessary;

Am I the only one who didn’t know that my HS yearbook photo, shown with my full name, birth year and city of residence in 2001 (the yearbook in question) was publicly available to anyone with an Ancestry account? Apparently there have already been lawsuits about it, but Ancestry has won them all. 😕
