This reveals a strength of constitutional monarchies/parliamentary systems: the trappings of state are embodied in the monarch, so the Prime Minister taking care of daily governance isn't typically worshipped like a god.
Conversely, strong Presidential systems are very vulnerable to strongmen.

There still has to be some way to control costs. Single payer systems still deny certain treatments or services, it's just a state bureaucrat instead of a corporate bureaucrat making the decision. Treatment may also be rationed through waiting times or reduced availability. No magic bullet here.
