One of my pet peeves is checking FOBT to confirm that someone is bleeding in the hospital setting. There is a high false positive and not what the test was intended for. It also doesn’t change management in most cases (I guess maybe unless negative but false negative can also happen).

One of the most satisfying things about medicine and particularly hospital medicine and critical care is seeing the immediate results of your efforts. Just makes it all worth it.

Not med related. Recently got into 3D printing & design but very proud of this one I made for my son due to the challenges I had to solve to design & print it.

Looking to do future models that are medicine related as well.

My 2024 was a year full of growth, learning, smiles and struggles. And while today is no different than yesterday, it’s still a good time to reflect, turn a page and look forward to more of life in 2025. Happy new year everyone.

- Leg cramps -> calcium 4.1, hemoglobin 6.6
- Flank pain -> obstructing ovarian mass causing hydroureter/hydronephrosis
- Abdominal bloating -> diffuse metastatic disease
- fever, spit out blood -> flu A with tonsillar hemorrhage

Fast track is a ~*trap*~

Don’t think it has a role in the inpatient side. Lots of false positives and doesn’t change my management plan. Perhaps a good negative test in specific circumstances but mainly should be used in outpatient for colon Ca screening.

Among critically ill adults who received invasive mechanical ventilation for more than 24 hrs, screening frequency (once-daily vs more frequent screening) & SBT technique (pressure-supported vs T-piece SBT) did not change the time to successful extubation.
