Pneumatic Retinopexy is far superior to a Vitrectomy to repair a detached retina.

On the other hand, I don't recommend one detached retina, much less two of them to be able to compare.

RESCUE PNEUMATIC RETINOPEXY FOR RETINAL DETACHMENT IN VITRECTOMIZED EYES: PnR in Vitrectomized Eyes
Purpose: To report the visual and anatomical outcomes of pneumatic retinopexy in vitrectomized eyes. Methods: This was a retrospective multicenter study evaluating eyes 1) treated with pneumatic retinopexy for rhegmatogenous retinal detachment (RRD) that 2) had a history of pars plana vitrectomy for any indication. At least 3 months of follow-up was required. Results: A total of 38 eyes from 38 patients met the inclusion criteria. Retinal reattachment after a single pneumatic retinopexy was achieved in 71.1% (27/38). The primary anatomical reattachment rate was 100% (12/12) for eyes those previously underwent pars plana vitrectomy for indications other than RRD, but lower (57.7%; 15/26) for those with a history of RRD repair (P < 0.001). The median logarithm of the minimum angle of resolution visual acuity (Snellen equivalent) improved from 0.74 (20/110) to 0.48 (20/60) at 3 months (P = 0.004), 0.44 (20/55) at 6 months (P = 0.002), 0.35 (20/45) at 12 months (P = 0.007), and 0.30 (20/40) at the final visit (P = 0.007). Conclusion: Pneumatic retinopexy may be a useful and less invasive rescue technique to reattach the retina in previously vitrectomized eyes with a RRD, especially in those without a RRD history.

We read with interest the article by Griffin et al.1 The authors present a large volume of registry data pertaining to pneumatic retinopexy (PnR) outcomes. Aggregated clinical data provides a potentially valuable counter to the rigor of the randomized control trial, but it must be viewed with caution. Although certain methodological limitations are acknowledged, we have observed important additional unaddressed confounders and incongruities that serve to weaken the authors’ conclusions.

Muni et al1 have shown that successful pneumatic retinopexy (PnR) promises excellent visual function for carefully selected patients. However, for patients who do not meet the strict inclusion criteria of the Primary Rhegmatogenous Retinal Detachment Outcomes Randomized Trial (PIVOT) study, PnR may be less likely to achieve single-operation success (SOS).2 Although a surgeon’s skill and preferences may influence which procedure they elect for rhegmatogenous retinal detachment (RRD) repair, patient-level factors are most influential in determining which type of RRD surgery is chosen in the United States (US).

PROPHYLACTIC LASER RETINOPEXY AND SCLERAL BUCKLE PLACEMENT FOR RETINAL DETACHMENT PREVENTION IN PIERSON SYNDROME: A Retrospective, Case Series Study
Purpose: To report the effect of prophylactic 360° laser retinopexy and scleral buckle placement for retinal detachment (RD) prophylaxis in patients affected by Pierson syndrome (PS). Method: This retrospective case series included 11 PS patients (2015–2024), analyzing demographic data, ophthalmological examinations, and genetic testing. Prophylactic interventions were compared with post-RD surgical interventions based on RD occurrence and timing. A one-sample proportion test evaluated outcome differences. Results: This study of 11 PS patients evaluated the effectiveness of prophylactic 360° laser retinopexy and scleral buckle placement. Ten patients carried the LAMB2 c.440A>G mutation and had associated findings of thyroid and renal disease. All RDs were combined tractional RD (TRD) and rhegmatogenous RD (RRD) due to posterior tears. Eight eyes with TRD-RRD were actively treated with laser retinopexy and SB; six (75%) experienced redetachments. By contrast, seven eyes with TRD-RRD were treated prophylactically, with 86.6% of eyes treated with laser retinopexy and 100% of those receiving additional scleral buckle placement maintaining retinal integrity over an average follow-up of 3.4 years. Prophylactically treated eyes had significantly fewer RD recurrences compared with those treated surgically after RD onset (P < 0.01), highlighting the benefits of early combined intervention in PS. Conclusion: This study's results suggest a potential role for prophylactic laser retinopexy and subsequent scleral buckle placement to prevent RD among patients with a confirmed diagnosis of PS.

Griffin et al1 evaluated pneumatic retinopexy (PnR) outcomes using the IRIS Registry. The authors reported single-operation success (SOS) rates relative to prior studies, including the Pneumatic Retinopexy versus Vitrectomy for the Management of Primary Rhegmatogenous Retinal Detachment Outcomes Randomized Trial (PIVOT), a randomized controlled trial comparing PnR with pars plana vitrectomy (PPV).2 They found a 59.8% SOS rate for PnR, which was considerably lower than the 81% achieved in PIVOT, concluding that anatomic outcomes with PnR in a clinical setting are inferior to those published in the randomized controlled trial.

Proof of how cat-brained I am: I just had to have a pneumatic retinopexy due to a retinal tear, and the gas bubbles in my eye have clustered into one big one and five small ones along one side of it. Yeah, it looks like a paw.
