This is your reminder that:

PT is a one to one treatment with a licensed therapist.

If you are in a federal program like Medicare, Tricare or veterans affairs: you must be seen by a licensed therapist at all times.

Me: anything new?

Patient: "nope"

Me: any improvement that you want to tell me about?

Patient: " oh, I'm walking better."

Sometimes you have to ask the direct question instead of only open ended.

The GOOD: exercise can reduce mortality with a cancer diagnosis.

The bad: exercise will not cure cancer.

The ugly: people continue to think we are hiding the cure for money.

We (they) are not.

The good: there is no such thing as muscle knots.

The bad: muscles do feel tight and painful in small areas.

The ugly: there are what feels like millions of massage, physical and myofascial therapists that will charge you money to tape, needle, deep tissue and suction cup imaginary knots.

Things I can't believe I forgot or didn't know after 23 years of being a PT: shanks are shins

The good: your body can heal all types of injuries with clasts and blasts. Cells that clean up and rebuild.

The bad: it will only do as much as it needs to to survive unless you ask it to do more. (Progressive overload)

1/2 The good:
Humans can prevent many lifestyle diseases with simple habits like exercise and eating a few extra healthy foods everyday.

The bad:
there are too many people that do not have access to healthy foods and do not know what exercise is best for them.

The bad...

Physical therapy is a place to push yourself to do more than you think you can while injured or in pain.

If you ever think: I could do this at home while in a PT session, you need to go find a new therapist.

Happy New Year!

New year good news:
Most rotator cuff (stability cuff) tears can get better without surgery and (with just simple exercise) have better outcomes with no adverse events two years after the start of pain.

My young old patient with left failed rotator cuff repair and full thickness right tear that couldn't lift her arms three weeks ago, just lifted 7#with two hands as a one rep Max.

This is how rehab can help even with tears and advancing age without surgery.

Recovering from any injury takes time and unfortunately, pain.

The good news is that most injuries will get better of you keep moving and build your injury back with simple exercise over 7 days to 2 years: simple ankle sprain versus frozen shoulder.

You don't need special exercises.

Everything you consume counts towards your water intake for the day.

Coffee
Meat
Salads
Fruit
Veggies
Cola

You don't need to drink 64 oz of water each day.

Especially if you eat your fruits and veggies.

Drink if thirsty.

Follow nutritionist orders if you have kidney ds or heart congestion

It's Sunday.

Do you rest, start your training week or is it the middle of the week for you?

Physical pain "root cause" is difficult to find.

Sometimes it's just staring us in the face: broken bones and irritated nerves.

But many times we don't need the cause and can't even find it with MRI.

Everytime we push past our physical and mental capacity with exercise, we adapt to make this the new normal.

If you stay within your capacity and never push past, it seems that we naturally do a little less which reduces strength, capacity, while increasing morbidity as we age.

Push yourself.

Aggravating Truth:

There is no one magic exercise to cure low back or any other pain.

Pain is not that simple and imagine if it was: nobody would need physical therapy, surgery or do more than that one exercise you saw on SM.

Motivation Monday:

The longer you lift weights and strengthen your entire body, especially your legs: The longer into older age you can get up from your chair, play with your grandkids and live life.

Lift weights for life.

Health is built with slow, boring consistent work and rest.

Resting from worrying about getting to the gym and eating right today is an important part of the programming.

Back pain cue of the day:

Relax your core.

Don't tighten it.

Breath and relax.

For those just starting to exercise to improve health:

Train to gain.

Gain muscle.

Gain conditioning

Gain fat.

Gain weight.

For a year.

Then cut some weight off if you still need to.

You probably won't need to buy then but that's when you can start.

When an electrical stimulation rep comes in hot with:

"It's like doing 20k sit ups in a session!"

The best results from physical therapy come from the trust built between the therapist and patient.

Things that help as well:

1 to 1 care without the use of techs.

Active therapy over passive

Creating Self-efficacy

Honesty in what PT can and cannot achieve.
December 10, 2024 at 8:44 PM
Acceptance of critical evaluation of my biased opinions as a PT is one of the biggest reasons I grow as a therapist.

Self-assessment and criticism of my beliefs is why I changed ten years ago.

"I didn't get to eat until I exercise"

Poor relationship with food.

"I exercised so I need to fuel appropriately"

Healthy relationship with food.

"I ate and now need to burn 110% of my calories with exercise."

Form of bulimia in need of treatment possibly.
