I think reframing the action from "confirming a crit" to "getting another attack" does alot to make it feel better imo. The former feels like the game taking something away you earned, while the latter feels like you are possibly gaining something more

A 2nd optional boss fight. The PCs will be bloody and bruised and if they choose to fight on they might die but change the world forever (more than they already have). This is sort of like the secret boss fight against God in tons of jrpgs

I really prefer the first one. Love the cohesive color pallet. Looks like it belongs on the shelf next to the White Wolf games in the 90s

The part of the “old school” worth reviving has never been THAC0 or fighting orcs in gray corridors but the DIY attitude of design that ceased once D&D grew from being just a Game to being a Brand

A nice middle ground I like is flash cards and minis. Especially dry erase flash cards. Helps you keep track of who's engaged and not, who/what's in each zone, etc. I haven't used this method in HMTW yet, but seems like it would translate flawlessly.
