the displacement forces on the condyle are pretty huge, and even NWB, any valgus knee stress is going to want to wreck my reduction, so the distal DCS screws were placed to not only engage and compress the fragment, but to also 'haystack' against the most distal transverse screw

This one distal posterior screw being the killer, and driving right through the meat of the fragment and scraping along the roof of the notch (crosshairs on the CT demonstrate the screw position/trajectory)

this is the only place (meaningful) transverse screws can go

“High ankle sprain” can be such a misnomer. I’ve never liked the term

Syndesmotic tears can lead to massive joint instability if not recognized and appropriately managed.

This case was a missed maisonneuve with a completely normal NWB ankle plain film
