The version I have is by Leon Rosselson (1973) and I have pictures but they are quite small (photocopies my dad did at work out of one of my mum's books I think). Not sure the book!

This is the screen, easy to tick yes to and I don't think it checks up on you. I get mine for epilepsy anyway but I don't think they would turn you away as I've never been questioned at the appointment.
