Independent providers like @queerdoc.bsky.social are stepping in as hospitals reduce or eliminate gender-affirming care, but this makes them easier to single out for harassment. It's a dynamic we've long seen in abortion care, as @garnethenderson.com notes

Beal’s practice had already seen an influx of new patients due to state restrictions even before Trump took office, they said. And in Illinois, a network of independent providers who don’t rely on federally-funded programs like Medicaid have stepped in to continue patients’ gender-affirming care as local hospitals have backed off. screenshot 2/2 Beal is proud of the work independent practices are doing to support their patients, but also worries it could lead to violence against providers. Clinicians are organizing, Beal said, “to continue care when these large systems close.” “But then it is falling more on individual clinicians to make those things happen,” Beal added. “Which, if you have care siloed, it is also much easier to target those clinicians’ physical safety.” This mirrors a dynamic that has long existed in abortion care. The upside of specialized clinics is expert care and mission-driven staff. The downside is that these practices are easier to single out, whether for harassment, violence, or government surveillance.

Here's an important one that slipped by on Monday.

Fed. judge quashes DOJ subpoena seeking the personal health data of trans patients as "prosecutorial coercion" and "for an improper purpose," namely, intimidating trans people and their medical providers.

This mismatch is not just a technicality. It suggests that DOJ issued the subpoena first and searched for a justification second. No legitimate investigation would demand thousands of patient records from an entity that cannot, by definition, commit the violations being investigated. DOJ’s inability to articulate why it is investigating QueerDoc specifically—beyond noting it is a “prominent” Case 2:25-mc-00042-JNW Document 27 Filed 10/27/25 Page 13 of 16 ORDER - 14 1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 provider—confirms that QueerDoc was targeted for what it does (provide genderaffirming care) rather than how it does it (through any unlawful means).2
