Pocket stuff isn't viable in most places in India due to legal regulations against sex selective abortion. But I'm very glad that this discussion is being initiated because I would get very annoyed on seeing everyone learning in workshops but the actual workspace can't implement it.

+they are providing any Anaesthesia with O2+Nitrous alone(with no propofol in any infusion). This treatment of Anaesthesiology services is evidence of a thoroughly broken/non existent system except for a few islands of standard practice/excellence.

+services have to be treated in the most niggardly/stepmotherly fashion possible and Anaesthesiologists in the system themselves having a certain Stockholm syndrome like attachment to that awful system and calling me a coward/untrained when I point out that one is Macgyvering at best if they think+

I was like physically uncomfortable as I saw them do it without them...it only needs one case that crashes to just put a stop to all this nonsense...Or maybe it won't? The unhappy post-junior residency tales I have to tell!

Also another subpoint regarding that:How to impart lessons about anaesthesia related topics in such compromised situations. If they are not used to attaching ECG leads for cases under spinal anaesthesia how will they practically realize what ASA I and II standards mean for anaesthetic monitoring?

Yeah,I was referring to myself in the third person. I also feel bad for my residents....I told them I feel physically uncomfortable if you don't attach the ECG's in a GA case so they do that under me now.

I think we need a thread on 'Anaesthesia in compromised environments due to inadequate equipment:Where the Anaesthesiologist cannot really say no because that's his whole environment'

It's been officially renamed to Vijay Durg(Victory Fort)...though I feel it will take a few more generations before that renamed name catches on since there was no big clamour from the civillian public(that being said they would not know who the William in Fort William was).

shyamasundaradasa.com/jyotish/reso... I would actually take the first breath considering my background. If the baby doesn't cry and the peds guys jump on to the case then there usually is something going to be wrong.
correct time of birth
Shyamasundara answers the question -what constitutes Time of Birth?.

Hip fractures concern elderly patients with physical and cognitive comorbidities. Mortality is high, 13% die within one month, 25% die within one year after the injury. International guidelines suggest that adequate pain management requires operative management even with a poor life expectancy.

People used to bite into lemons (also relatively modern) like they were candy. Can you imagine expecting a lemon's tartness & getting an orange's sweetness?
