We do not routinely biopsy but we are doing it more often than before.

I am big proponent for biopsy before systemic therapy as wrote in this review pmc.ncbi.nlm.nih.gov/articles/PMC....

But I still do not think the evidence is strong enough to make it into the guidelines.

Interesting, what was the GGT? I haven’t seen this in my practice, but apparently can happen rarely, here is a nice study pubmed.ncbi.nlm.nih.gov/33229138/

Why not treat all the large varices? Is the idea to decrease risk of post banding ulcer bleed? And unless it’s actively spurting at the time of scope, it’s not always obvious which the bleeder is, right?
