Thx for sharing our endTB results. Too bad u weren’t able to include my comment on the chilling effect on medical advances in TB, HIV, cancer, heart disease, etc. wrought by current US leadership. endTB was possible only bcuz of trust cultivated over decades.½

Though "Oral is the New IV" with antibiotics (h/t @bradspellberg.bsky.social), it's not always easy to convince reluctant inpatient clinicians -- especially (in my experience) surgeons, who want to treat "aggressively".

Bedaquiline is an amazing drug for tuberculosis, greatly shortening DR-TB treatment

But it also works for leprosy (another ancient mycobacterial infection)!

In patients with multibacillary leprosy, bedaquiline cleared M. leprae in 4 weeks

While attention is trained on the potential infectious cause which could spread internationally, we should not ignore that food insecurity, malnutrition and lack of access to basic health needs seems to be the major driver of bad outcomes here. All social determinants of health enhanced by inequity.
