A year into this administration, decades of bipartisan progress on global health have been undone. Billions in foreign aid were cut or withheld, hurting communities around the world.

Here are five key reads from the past year on why these cuts matter: www.pih.org/article/unde...

Acefie Théodore was diagnosed with HIV in 2008. Despite long walks to care, she stayed committed to treatment.

In 2023, Zanmi Lasante clinicians found her bedridden and rushed her to care, where she was diagnosed with TB. With comprehensive support, she survived.

Cutting U.S. foreign aid is fueling a global health crisis, but that’s only the surface.

The real problem? An aid system built on dependency, not justice.

A year after U.S. foreign aid cuts, the good, the bad, and the ugly tell one story: global health decisions cost lives.

Behind every foreign aid cut is a patient, a nurse, a family. The damage is real, but so is the fight to protect health as a human right.

This targeted attack on marginalized and under-served populations by the Trump Administration will prevent care, services, and support from reaching people who need them most.

PIH calls for the immediate and permanent repeal of the Global Gag Rule.

5/ Furthermore, two new rules were issued that prohibit U.S. foreign assistance from being used in any diversity, equity, and inclusion initiatives or promoting what the administration calls “gender ideology,” which will inevitably harm communities.

4/ The updated rule, issued by the State Department on January 23, 2026, expanded the rule to apply to all non-military U.S. foreign assistance and to include international organizations, U.S.-based NGOs operating abroad, and foreign governments.

3/ This updated rule, cloaked in humanitarian language, will weaken already fragile health systems and profoundly undermine the fundamental rights of women, disproportionately impacting communities like those PIH serves.

2/ PIH strongly opposes the President and the U.S. State Department's reinstatement and expansion of the Mexico City Policy, known as the "Global Gag Rule." By expanding the impact of restrictions on abortion, these changes further compromise patient-centered, quality reproductive health care.

Diseases like cholera, TB, HIV, and malaria are treatable. Yet millions still die, not because cures don’t exist, but because access does.

Health care is a human right. And in the 21st century, preventable diseases should never be a death sentence.
